Application Requirements

For Sole Proprietors, Partnerships and Corporations 

  • Duly filled out ORIX METRO Credit Application Form
  • Audited Financial Statements with Income Tax Return (ITR) for the past three (3) years
  • For Corporations: SEC Registration, Articles of Incorporation, By-Laws and General Information Sheet (3 years)
  • For Partnership: SEC Registration, Articles of Partnership and Partner's Resolution
  • For Sole Proprietorship: DTI papers
  • Business permit
  • Updated company profile
  • Latest interim financial statements
  • Photocopy of two valid IDs of signatories and corporate secretary
  • Copy of unit price quotation from supplier, if available

For Individual Accounts

  • Duly filled out ORIX METRO Credit Application Form
  • Two (2) copies of latest 2" x 2" colored I.D. picture
  • Photocopy of company I.D.
  • Photocopy of any valid government-issued I.D.
  • Certificate of employment from employer indicating status of compensation, designation, position and tenure
  • Certified true copy of monthly payslip for the past three (3) months
  • Latest Income Tax Return (ITR)

 

List of Acceptable/Valid Identification Documents (IDs)

Any one (1) of the following:

  1. PhilID or ePhilID
  2. Passport including those issued by foreign governments
  3. Driver’s License
  4. Professional Regulations Commission (PRC) ID
  5. NBI Clearance
  6. Police Clearance
  7. Postal ID
  8. Voter’s ID
  9. Tax Identification Number (TIN)
  10. Barangay Certification
  11. GSIS e-card / Unified Multipurpose ID (UMID)
  12. SSS ID
  13. Senior Citizen Card
  14. Overseas Workers Welfare Administration (OWWA) ID
  15. OFW ID
  16. Seaman’s Book
  17. Alien / Immigrant Certification of Registration
  18. Government Office and GOCC ID (e.g. AFP, HDMF IDs)
  19. Certification from National Council for the Welfare of Disabled Persons (NCWDP)
  20. Department of Social Welfare and Development (DSWD) Certification
  21. Integrated Bar of the Philippines (IBP) ID
  22. Company IDs issued by private entities or institution registered with or supervised,  or regulated either by the BSP, SEC or IC
  23.  Student / School IDs signed by the school principal or head of the educational institution
  24. SEC Certificate of Registration
  25. Business Registration Certificate
  26. PhilHealth ID
